D3400 blurred face

I'm novice and have been fiddling with D3400. For some reason most of the time a person's face comes out to be blurred. It's not too much blurred but if you zoom in then you can see it clearly. I've tried cleaning lens, using auto mode, low ISO value but no luck. There is no shaking, there is sufficient light, tried with flash without flash.

My lens is 35mm 1:1.8G.

Any suggestions on how to fix it?

Upload an example, make sure it is only 1024 pixels on the longest side and less than 250kb.

You need to upload an image and also tell us the shutter speed, aperture, and focusing mode that you used.

If you shoot NEF (RAW) then I would suggest uploading a NEF file to dropbox or somewhere similar and sharing the link here.
